GENERAL FUNCTION: Assist Providers with Patient follow up.

DUTIES AND RESPONSIBILITIES INCLUDES, BUT IS NOT LIMITED TO:

· Monday through Friday 8am -5pm

· Point of contact for providers patients

· Send in patient prescriptions

· Working with local agencies to reach and contact patients

· Mailing, Scanning and faxing documents

· Dependable, punctual and a team player

· Any other duties as assigned

Education, Training, Knowledge and Experience:

• Bachelor of Science degree in nursing, nursing administration from an accredited college or university, or an Associate degree, or diploma in nursing from an accredited school of nursing
• State Nursing License
• Two years experience preferred but not required
• State Nursing License
• Basic computer skills
• Previous supervisory experience preferred but not required
