Full Job Description

Position Summary:

Responsible for collection of outcomes assessments and patient satisfaction surveys, communication with the treatment team and documentation of completion.

Position Responsibilities:

Clinical / Technical Skills (40% of performance review)

  • Assist the patient with completion of admission and discharge outcomes assessments appropriate to population and program within the designated time frames.
  • Assist the patient with completion of patient satisfaction surveys.
  • Notification to treatment team members of completion of admission and discharge outcomes assessments and communication of identified patient safety issues to treatment team.
  • Ability to use the dynamic forms for completion of outcomes assessments and satisfaction surveys.
  • Ability to scan and file completed outcomes assessments in the electronic health record.
  • Provide data of completed outcomes assessments and patient satisfaction surveys including patient comments.
  • Understanding of the Discharge Calendar for preparation for discharge and to ensure completion of discharge outcomes assessments and patient satisfaction surveys are completed and reviewed prior to discharge.
  • Interdepartmental communication to ensure admission outcomes assessments are completed within the designated time frames.
  • Assist the patient in registration and completion of additional engagement programs.
  • Perform other duties as required.

  • Participates in quality projects, as assigned, the monthly Performance and Improvement Committee and supports quality initiatives.

  • Builds collaborative relationships with patients, families, staff, and physicians.
  • The ability to retrieve, communicate, and present data and information both verbally and in writing as required

  • Demonstrates adequate skills in all forms of communication.

Education

  • Required: High School Diploma or equivalent. CPR and hospital-selected de-escalation technique certification.
  • Preferred: Some college courses

Experience

  • Preferred: One (1) year of experience in a health care or behavioral health care setting.
  • May substitute education for experience.
