Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a Full time Community Outreach Worker Level 1 to join our team in Monticello, New York.
We are seeking an outgoing dynamic Community Outreach Coordinator to engage our communities in need at our Monticello site with travel as needed throughout Hudson Valley.
In conjunction with departmental staff, work towards retaining patients by contacting those in need of follow up. Engage patients so they are aware of you as a reliable face of the health centers that you serve. Identify patients who have been individually contacted but not yet converted to patients and follow up with telephone or other communication as appropriate.
Responsibilities:
- Health promoters – organize community volunteers to receive SRH Training in order to provide lay health education and outreach to community.
- Provides ongoing social and emotional support through care management.
- Attends in-service education and training programs.
- Assists in obtaining essential support services such as Medicaid, food stamps, transportation, translation services and day care.
- Completes daily activity logs, referral forms and other reports, as necessary.
- Arranges referrals and provides transportation as needed.
- Serves as an advocate for the patients, and acts as a liaison between the patients and community agencies.
- Assists medical staff in care management through outreach using patient registries and medical interpretation.
Requirements:
High school diploma or equivalent and Driver's License required.
Associate's degree preferred.
Minimum 6 months outreach experience preferred.
