Job Responsibilities:

• Receive all incoming external referrals by fax, phone, or email.
• Contact patient for scheduling within 24 hours.
• Acknowledge patient referral by contacting the external provider.
• Inform Manager/COO of increase or decline of any referral source.
• Maintain an accurate database of all referral sources
• Actively research new referral resources.

• High School Diploma or GED required.
