About the Job

As an RN Health Coach, you'll partner with patients to help them better understand their health and achieve their wellness goals through education, coaching, and ongoing support. Using your nursing knowledge and strong communication skills, you'll empower patients to make sustainable lifestyle changes while collaborating closely with providers and other members of the care team.

What You'll Do

  • Provide individualized health coaching, education, and support to help patients achieve their health goals

  • Guide patients in managing chronic conditions through evidence-based education and coaching

  • Educate patients on disease processes, medications, treatment plans, and preventive care

  • Review health assessments, biometric screenings, laboratory results, and other clinical data to identify opportunities for improved outcomes

  • Utilize motivational interviewing and behavior change techniques to encourage healthier lifestyle choices

  • Collaborate with providers and interdisciplinary team members to support coordinated, patient-centered care

  • Document patient interactions, coaching activities, and progress within the electronic medical record

What We're Looking For

  • Current, active Registered Nurse (RN) license in the state of practice

  • Bachelor's degree preferred

  • Minimum of two years of experience in chronic disease management, care management, health coaching, patient education, population health, or a related clinical setting

  • Strong clinical assessment, communication, and patient education skills

  • Passion for helping patients improve their health through education, coaching, and relationship-based care
