About the Job
We are looking for a compassionate and detail-oriented LPN Intake & Care Coordination Nurse to support primary care providers through patient intake, care coordination, clinical follow-up, and administrative nursing functions.
In this role, you'll help improve continuity of care by gathering patient information, coordinating follow-up services, supporting preventative care initiatives, managing routine clinical communications, and ensuring patients remain engaged in their care plans.
What You'll Do
Conduct comprehensive patient intake assessments and gather medical, medication, family, and social histories
Review, update, and maintain patient information within the EMR
Prepare charts and documentation for provider review prior to appointments
Conduct patient follow-up regarding lab results, diagnostic testing, and provider-directed treatment plans
Coordinate referrals, specialist appointments, medical records, and follow-up care activities
Monitor outstanding labs, imaging studies, referrals, and care plan activities
Assist providers with clinical inbox management, patient messages, and prescription refill requests
Complete prior authorizations and coordinate supporting documentation
Facilitate communication between patients, providers, pharmacies, specialists, and healthcare organizations
Support chronic disease management, preventative care outreach, and population health initiatives
Maintain accurate documentation and ensure compliance with clinical protocols and HIPAA requirements
Support wellness events, health assessments, and other patient outreach activities as needed
What You'll Bring
Graduate of an accredited Practical Nursing program
Current and unrestricted Licensed Practical Nurse (LPN) license
Current BLS certification required
Minimum two years of nursing experience in primary care, family practice, or internal medicine required
Experience utilizing electronic medical record (EMR) systems required
Care coordination experience preferred
Experience with patient intake, referrals, prior authorizations, and clinical documentation preferred
Strong knowledge of primary care workflows, chronic disease management, preventative care, and patient care processes
Understanding of referrals, prior authorizations, and insurance processes
Excellent communication, customer service, organizational, and time-management skills
Ability to manage multiple priorities while maintaining accuracy and attention to detail
