About the Job

As an RN Health Coach, you’ll partner with patients to support sustainable lifestyle changes, improve chronic conditions, and build confidence in managing their health.

You’ll combine your clinical expertise with coaching techniques to guide patients through behavior change, helping them better understand their health and take an active role in their care.

This is a proactive, education-focused role where you have the time to connect, coach, and make a lasting impact.

What You’ll Do

  • Provide personalized health coaching and education to support patient goals

  • Guide patients in managing chronic conditions through lifestyle and behavior change

  • Use motivational interviewing and goal setting to support sustainable outcomes

  • Educate patients on disease processes, medications, and preventive care

  • Collaborate with providers and care teams to support coordinated, whole-person care

  • Track patient progress and document coaching interactions in the EMR

What You’ll Bring

  • Licensure: Current Registered Nurse license in the state of practice

  • Experience: 2+ years of experience in patient education, chronic disease management, or care coordination

  • Education: Bachelor’s degree in Nursing (preferred)

  • Skills: Strong communication, relationship-building, and motivational interviewing skills
