About the Job:

Picture walking into a welcoming primary care clinic, ready to utilize your specialized nutrition expertise to empower individuals on their journey to better health. Your day involves providing personalized nutrition assessments and developing tailored dietary plans to manage chronic conditions and promote overall wellness. You'll collaborate closely with physicians and other healthcare professionals, playing a vital role in the holistic care of our patient community.

What You’ll Do (Key Responsibilities):

  • Nutrition Assessment & Planning: Provide comprehensive nutrition assessments and develop individualized nutrition care plans, utilizing evidence-based nutrition guidelines.

  • Counseling & Education: Deliver expert nutrition counseling to patients, addressing a variety of health conditions, particularly chronic diseases. Educate patients on therapeutic diets, meal planning, and healthy eating habits.

  • Interdisciplinary Collaboration: Collaborate with an interdisciplinary treatment team (including physicians) to integrate nutrition interventions into patient care.

  • Documentation & QI: Track patient progress and document interventions effectively. Contribute to clinic support and ongoing education initiatives related to nutrition and wellness.

What You’ll Bring to the Team (Requirements):

  • Licensure & Education (Required):

    • Registered Dietitian (RD) credential from the Commission on Dietetic Registration (required).

    • State License if required in the practice state.

    • Bachelor’s degree in Food and Nutrition, Dietetics, Nutritional Sciences, or Human Nutrition.

  • Experience & Skills:

    • Strong knowledge of clinical nutrition and disease management.

    • Excellent communication, counseling, and interpersonal skills.

    • Ability to work effectively within a collaborative, interdisciplinary team.

  • Preferred:

    • Minimum of one year experience providing nutrition counseling in an outpatient setting preferred.

    • Relevant specialty certification (CDCES, CSOWM, etc.) preferred.

    • Master's degree preferred.
