About the Job:

Imagine stepping into a welcoming primary care clinic, ready to connect with individuals motivated to make positive health choices. Your day involves engaging in meaningful one-on-one conversations, understanding each patient's unique journey, and collaboratively setting achievable wellness goals. You'll guide individuals to incorporate regular physical activity, make informed nutritional choices, and develop effective strategies for managing stress. As an integral part of an interdisciplinary team, you'll contribute your expertise to empower patients in their chronic disease management and overall well-being.

What You’ll Do:

  • Work collaboratively with an interdisciplinary treatment team to provide healthy lifestyle education and behavioral change support.

  • Provide individualized coaching and guidance to patients on nutrition, physical activity, stress management, and other wellness topics.

  • Assist patients in developing and implementing personalized healthy lifestyle plans.

  • Support patients in managing chronic conditions through education and ongoing encouragement.

  • Contribute to clinic support and ongoing education initiatives related to health and wellness.

  • Track patient progress and document interventions effectively.

  • Stay current on best practices in health coaching and behavioral change strategies.

What You’ll Bring to the Team:

  • Bachelor’s degree in a related health field (preferred).

  • 1 year of experience in Health Promotion, Wellness, or Fitness Programming (required).

  • State licensure (if required by practice state).

  • Certification in Health Education (CHES) or Health Coaching (CHWC) (preferred).
