About This Opportunity: CareATC is looking for an experienced RN Case Manager with strong communication skills to collaborate with our health center team to provide platinum level patient care and excellent customer service. The Registered Nurse Case Manager is a central member of the Total Health Solutions team, organizing patient care and acting as a liaison between our Clinical Providers and other healthcare providers to ensure a successful treatment plan for patients. They provide one-on-one advocacy, chronic disease management, case management, care coordination and assist in treatment plan implementation. They work closely with physicians, health center staff, clinical providers and other members of the Total Health Solutions team coordinating patient care and ensuring team communication and collaboration. The Registered Nurse Case Manager develops care plans; coordinates between patients, care providers, and facilities to execute the care plan; ensures patients and caregivers understand the treatment options and communicate with them regularly. They are expected to identify, communicate with, and follow up with all referral agencies on a regular basis, assisting patients with connecting with available benefits on their health plan.

This is a full-time, 40-hour-per-week, benefit-eligible opportunity in Louisville, KY. Monday-Friday. No Nights or Weekends!

The role of the RN Case Manager:

  • Conducting initial comprehensive assessments for patient’s needs and support systems and assisting in creating a personalized plan.

  • Provides clinical education on risk prevention to patients and their families.

  • Evaluates and assists patients with medication and care plan compliance.

  • Helping patients make informed decisions by acting as advocates regarding treatment options

  • Must have knowledge and experience in interpreting lab values and clinical measures in completing an accurate patient assessment. Experience in motivational interviewing preferred.

  • Efficiently and accurately charts in EMR system for patient interactions and care plans. Tracks and audits charts for compliance with care plans and screening opportunities, assisting medical providers with highlighting discrepancies in high-risk populations.

  • Strong organizational, communication, flexibility, and time management skills are required.

  • Comfortable working with health plans; assisting patients in connecting to benefits available to them.

  • Must be able to work independently, exhibit critical thinking, and prioritize tasks to meet commitments that are aligned with department goals.

  • Strategic thinker that can analyze problems, prioritize solutions, and manage multiple complex situations, while collaborating with team members as required.

  • Strong telephonic skills a must. Conducts clinical outreach to multiple patients that utilize the health center. Empowers patients to self-manage their health and chronic conditions.

  • Provides clinical support, as needed, to all members of the health center. Effectively collaborates and integrates with Operations, Medical and Total Health Solutions team.

  • Utilizes various technology platforms to identify high-risk populations for outreach.

  • Stays up to date on clinical best practices to provide relevant and effective clinical education programming.

  • Conducts large telephonic outreaches to encourage the patient population to stay engaged in health center services

  • Must be comfortable creating and giving presentations virtually to clients and administrators

  • Assist with triaging patient portal messages, coordinating with medical providers as necessary.

  • Assist with creating educational handouts for patients/company

  • Referrals- Ability to coordinate with outside providers, specialists, etc.

  • Other duties as assigned.

Qualifications

Education:

  • Bachelor’s degree in Nursing from an accredited university.

Experience:

  • Registered Nurse, Multi-state license required. License must be current and in good standing.

  • Minimum 5 years of working in healthcare setting or similar setting. Working knowledge of health insurance plans, electronic medical records, and Microsoft Office platforms.

  • Must possess a current CPR certification and current health records with the appropriate immunizations to work in the health care field (hepatitis B and tuberculosis).

  • Must be able to work independently, exhibit critical thinking, and prioritize tasks to meet commitments that are aligned with department goals.

  • Strategic thinker that can analyze problems, prioritize solutions, and manage multiple complex situations, while collaborating with team members as required.

  • Strong communication skills.

  • Previous experience as a Registered Nurse Case Manager in a hospital setting preferred
