About This Opportunity: CareATC is looking for a Registered Nurse to join our team in St. Louis, MO for a full-time, benefit-eligible opportunity. The RN Market Resource is a central member of the Clinical team and provides one-on-one advocacy, chronic disease management, case management, care coordination, and assists in implementing treatment plans. The RN Market Resource is responsible for conducting outreach to identified patients and collaborates with all members of the Clinical team to provide clinical guidance, education, and program oversight. This role works closely with physicians, health center staff, ancillary providers, and other members of the Clinical team to coordinate patient care and ensure effective team communication and collaboration. In addition to chronic disease management, the RN Market Resource will also assist clinicians as needed with triage, lab reviews, patient intake, and patient outreach.

Schedule: Monday - Friday 8:00 am - 5:00 pm

Location: 2315 Dougherty Ferry Rd, St. Louis, MO 63122

Travel between health centers in the St. Louis market is required. Mileage is reimbursed between health centers.

The role of the RN Market Resource:

  • Clinically assesses patients. Utilize working knowledge and experience to interpret lab values and clinical measures for accurate patient assessments. Experience in motivational interviewing preferred.

  • Efficiently and accurately charts in EMR system for patient interactions and care plans. Tracks and audits charts for all enrolled patients for programs. Provides updates to management in a timely fashion, when requested.

  • Leads, organizes, and documents interdisciplinary team meetings with assigned Health Center staff and ancillary service providers. Strong organizational, communication, flexibility, and time management skills are required.

  • Ability to work independently, exhibit critical thinking, and prioritize tasks to meet commitments that are aligned with department goals.

  • Strategic thinker that can analyze problems, prioritize solutions, and manage multiple complex situations, while collaborating with team members as required.

  • Strong telephonic skills a must. Conducts clinical outreach to multiple patients that utilize services. Empowers patients to self-manage their health and chronic conditions.

  • Provides clinical support, as needed to all members of the Clinical team and ancillary service providers.

  • Effectively collaborates and integrates with Operations, Clinic Staff, and other departments.

  • Utilizes various technology platforms to identify high-risk populations for outreach.

  • Stays up to date on clinical best practices to provide relevant and effective clinical education programming.

  • Conducts telephonic outreach to enroll patients into programs.

  • Needs to be comfortable creating and giving presentations virtually to patients, clients and administrators.

  • Assist with creation and implementation of programs and initiatives.

  • Assist with creating educational handouts for patients/company.

  • Referrals-Ability to coordinate with outside providers, specialists, etc.

  • Other duties as assigned.

Qualifications

  • Registered Nurse, Multi-state license required.

  • License must be current and in good standing.

  • Bachelor’s degree in Nursing from an accredited university.

  • Minimum 5 years of experience working in healthcare or similar setting.

  • Working knowledge of health insurance plans, electronic medical records, and Microsoft Office platforms.
