About This Opportunity: CareATC is currently looking for a Health Coach to work collaboratively with our primary care and interdisciplinary care teams to provide nutrition counseling, exceptional patient care, and achieve optimum health in a clinical setting. The CareATC approach to is to work with our patient population to assist in chronic disease management through individual interventions, clinic support and ongoing education.

This is a full time, 32 hour per week, benefit eligible role in Albany, GA. CareATC is seeking a Health Coach for an onsite role starting around June 2024. No nights or weekends!

The role of the Health Coach:

• Use motivational interviewing to assist patients with behavior change.
• Facilitate wellness education programs to members.
• Collaborate with other team members to develop health education curriculum for site.
• Present on a variety of health education topics to larger groups (in-person and/or webinar).
• Conduct telephonic and electronic outreach to at-risk members to increase service engagement and health center utilization (if applicable).
• Additional duties as assigned.

• Associate or Bachelor's Degree in related field (e.g., fitness, health promotion, nutrition) preferred
• Certification in health coaching or related field is desirable
• Experience in health coaching or related field preferred
• Strong knowledge of behavioral health principles and motivational interviewing
• Experience with public speaking, Must feel comfortable presenting to business groups.
